REGISTRATION TRAY TMRW-SPC-CUS-006 • VERSION: 01 • IMPLEMENTATION DATE: DD MMM YYYY INSTRUCTIONS FOR USE TMRW REGISTRATION TRAY TMRW REGISTRATION TRAY Cryopreservation Identification and Organizational Device DEVICE DESCRIPTION The TMRW Registration Tray is an RFID registering site for cryodevices and their associated CryoBeacons. INTENDED USE To provide users with an RFID enabled tool for use during the registration of containers. INTENDED USERS The intended users of the Registration Tray are laboratory professionals including embryologists, andrologists, clinicians, laboratory technicians, and laboratory management personnel. Patients do not interact with or come into contact with TMRW products. The patient population served is men and women, typically age between the ages of 18 years to 50 years at the time of fertility tissue collection. CONTRAINDICATIONS None. ADVERSE EFFECTS None. STORAGE Store at room temperature. DISPOSAL Discard in a e-waste facility. There are no lithium batteries used to operate, nor are any toxic components used in the manufacturing of the Registration Tray. The PCB assembly is lead-free. Always use appropriate personal protective equipment (PPE) and follow your laboratory’s standard operating procedures and safety guidelines when working with liquid nitrogen (LN 2 ). The Registration Tray requires electricity to function. Use standard caution and care around electrical connections and cords to prevent bodily harm from electrical shock or damage to the machine. Always use proper power and proper electrical compliant connections in accordance with the local electrical code. Place the CryoBeacon in the leftmost slot in the Registration Tray. Label the cryodevices. Place the labelled cryodevices in the paired CryoBeacon. Cap the CryoBeacon. (a) For a Class A digital device or peripheral, the instructions furnished the user shall include the following or similar statement, placed in a prominent location in the text of the manual: Note: This equipment has been tested and found to comply with the limits for a Class A digital device, pursuant to part 15 of the FCC Rules. These limits are designed to provide reasonable protection against harmful interference when the equipment is operated in a commercial environment. This equipment generates, uses, and can radiate radio frequency energy and, if not installed and used in accordance with the instruction manual, may cause harmful interference to radio communications. Operation of this equipment in a residential area is likely to cause harmful interference in which case the user will be required to correct the interference at his own expense. 5. FCC RADIATION EXPOSURE STATEMENT: This device complies with FCC RF radiation exposure limits set forth for an uncontrolled environment. This transmitter must not be co-located or operating in conjunction with any other antenna or transmitter. The equipment should be installed and operated with minimum distance of 20cm between the radiator and your body. 6. ISED STATEMENTS This device contains license-exempt transmitter(s)/receiver(s) that comply with Innovation, Science and Economic Development Canada’s licence exempt RSS(s). Operation is subject to the following two conditions: (1) This device may not cause interference. (2) This device must accept any interference, including interference that may cause undesired operation of the device. The Ferrite was added during testing for EN/IEC 60601-1-2 testing to improve radiated emission levels and to improve the response of the EUT for conducted immunity. The ferrite bead shown in the pictures was not used nor was it required to demonstrate compliance to FCC and ISED regulations during transmitter testing which was performed before the EMC testing. However, the ferrite bead shown or an equivalent solution will be used in the final product. The Ferrite bead used was: Wurth Elektronik ferrite # 742 711 31 (2 turns)
