Exhibit I: Operational Description FCC ID: CM676A91341-600 Operational Description The 91341-05 is a battery-powered portable electrocardiogram (ECG)-only transmitter operating on a 25 kHz channel scheme in the 608 – 614 MHz band. The Telemetry Transmitter is a low-power device that is worn continuously by a patient. Patient physiological parameters are monitored and transmitted via radio frequency (RF) to a fixed antenna network. The antenna network feeds a receiver, which processes the data and forwards it to a central monitor for display. The telemetry transmitter is intended to be used on the orders of a physician and limited to the confines of a healthcare facility. It is not for vehicle use outside the healthcare facility. The 91343-05 is a battery-powered portable multi-parameter (ECG and SPO2) transmitter and the 91347- 05 is a wideband electrocardiogram (ECG)-only transmitter. Both operate on a 50 kHz channel scheme in the 608 – 614 MHz band. The 91343-05 is also equipped with an Infrared (IR) interface that accepts data from compatible devices to forward other patient information to the central monitor. The Telemetry Transmitters are low-power devices that are worn continuously by a patient. Patient physiological parameters, such as heart rate and pulse oximetry, are monitored and transmitted via radio frequency (RF) to a fixed antenna network. The antenna network feeds a receiver, which processes the data and forwards it to a central monitor for display. The telemetry transmitter is intended to be used on the orders of a physician and limited to the confines of a healthcare facility. It is not for vehicle use outside the healthcare facility. Exhibit R: Justification for Emission Designator FCC ID: CM676A91341-600 CM676A91341-600 Justification for Emission Designator Emission Classification Per 47 CFR 95.1115 (c), the EUT complies with the requirement that “a wireless medical telemetry device may transmit any emission type appropriate for communications in this service, except for video and voice. Waveforms such as electrocardiograms (ECGs) are not considered video.” The EUT has F1D emission. The EUT uses a digital, frequency shift keying modulation scheme with no sub-carriers. The emission designator “F1D” was selected based upon the guidelines in 47 CFR 2.201: “F” designates an emission in which the main carrier is frequency modulated. “1” designates a single channel containing digital information without the use of a modulating sub-carrier (the applicant confirmed that no sub-carriers are used). “D” designates data transmission, telemetry. As detailed in the user manual, the device is used to transmit non-voice, non-video, biomedical telemetry. Necessary Bandwidth Per 47 CFR 2.202(c), the necessary bandwidth is calculated using Carson’s Rule: 2M + 2D = 2(6140) +2(3000) = 18.28 kHz. Where “M” is equal to the EUT’s maximum modulation frequency in Hertz, and D is equal to the EUT’s peak frequency deviation (i.e. half the difference between the maximum and minimum values of the instantaneous frequency). Per 47 CFR 2.202(b), the necessary bandwidth is expressed “by three numerals and one letter. The letter occupies the position of the decimal point and represents the unit bandwidth”. The EUT's necessary bandwidth is expressed as “18K3” Hence the EUT’s emission designator is “18K3F1D”.
Exhibit F: Tune-up Procedure FCC ID: CM676A91341-600 Only information regarding operation at 608-614 MHz applies. Transmitter Tune-up Procedure at a specified power range. The 600MHz and 1400MHz transmitters are designed to operate at one power level and within the RF output power limits shown in the table below. There are no adjustments for changing the RF output power level either in the factory or by the end user. Subject Specifications Comments Frequency Range 600 MHz model 608.0 – 614.0 MHz 1400 MHz model 1395 – 1400 MHz 1427 – 1432 MHz 1400 – 1427 is a restricted band. Radio should never transmit into this band or outside the bands normally operated. Introduction This document details how the RF deck is controlled and operated. There are 2 high level functions: • Power up • Programming the synthesizer These high level functions are presented below. 5. Power Up The RF deck operates autonomous to the rest of the unit. To power up the unit all that is required is connection of a 9V battery, the unit will automatically set all it’s conditions to transmit within a certain amount of time, defined in the technical specification. It is designed in this way to provide live data transmission only, no other mode of operation exists. RF Control and OperationPage 2Printed: 13/08/01 2:53 PM Murandi Communications Ltd.Suite 240, 6715 - 8th St. NE, Calgary, Alberta, Canada, T2E 7H7 Tel: (403) 777-9988Fax: (403) 777-9989 5.1 Invalid Channel When reading the channel selection from the Dip-switches the microcontroller will perform a check for validity. On the condition that the settings are invalid the microcontroller will program unlockable data into the synthesizer, preventing transmission on an unknown channel. During power up the microcontroller reads and stores the Dip-switch settings on board, no changes to the switches following this will affect transmit in any way, until the unit is powered off, then back on. In the event of a reset or loss of Lock Detect, the microcontroller will reload data stored internally in FLASH memory and reprogram all possibly affected devices. In the event that the synthesizer registers programming changes during operation, it is expected that the synthesizer will lose lock at least momentarily if not permanently. This will prompt the microcontroller to reset and reprogram the synthesizer. 6. User Channel Selection Programming the synthesizer takes place each time the PWT is powered up. This operation is automatically completed by the microcontroller to enable the correct frequency for transmission. The required data to program the synthesizer is translated from the two – 8pin dipswitches (in the form of a channel selection) to the necessary code for frequency setting. This operation takes place only when the unit is powered up. For the switch settings to take effect the unit must be powered off, then back on. The following tables illustrates the switch settings for each channel and the corresponding frequency. 600MHz Band (WideBand)SW2SW1600MHz Band (NarrowBand)SW2SW1 ChannelRF Frequency (MHz)432187654321ChannelRF Frequency (MHz)432187654321 2242608.03751000110000101241608.0125010011011001 2244608.08751000110001001242608.0375010011011010 2246608.13751000110001101243608.0625010011011011 2248608.18751000110010001244608.0875010011011100 2250608.23751000110010101245608.1125010011011101 2252608.28751000110011001246608.1375010011011110 2254608.33751000110011101247608.1625010011011111 2256608.38751000110100001248608.1875010011100000 2258608.43751000110100101249608.2125010011100001 2260608.48751000110101001250608.2375010011100010 2262608.53751000110101101251608.2625010011100011 2458613.43751001100110101470613.7375010110111110 2460613.48751001100111001471613.7625010110111111 2462613.53751001100111101472613.7875010111000000 2464613.5875100110100000147361…

RF Exposure statement provided by client: This section is not applicable. The FCC, with full knowledge that patients within a healthcare facility wear these devices continuously in close proximity to the body, determined field strength limitations on transmitters operating in the Wireless Medical Telemetry System (WMTS). Based upon information provided by the FCC, using the rule-of-thumb that SAR~P, for all P transferred into 1g then 1.6 mW/g requires 1.6mW, which is above the output of the device described in this certification application.
