TEL + 81-857-21-2100 FAX + 81-857-21-2642 AFFIDAVIT FOR ESN PROTECTION OF CELLULAR MOBILE TELEPHONES We hereby certify that the Handheld Portable Cellular Telephone,FCC ID: NRNA5514SA is so designed that it complies with all the requirements for ESN protection specified in Section 22.919. a) The transmitter in service has a unique ESN. b) The ESN host component is permanently attached to a main circuit board of the mobile transmitter and the integrity of the unit operating software cannot be altered . The ESN is plated from fraudulent contact and tampering. The ESN is encoded using multiplication by a polynomial and the ESN data programmed in the memory with other information. c) The ESN is factory – set and cannot de altered, transferred, removed or otherwise able to be manipulated. Cellular mobile equipment is specifically designed such that any attempt to remove, tamper with, or change the ESN chip, its logic system, or firmware originally programmed by the manufacturer will render the mobile transmitter inoperative. Question #1: In the theory of operation, this phone is designed to have max. .25 W ( 24 dBm ) +2/-4 dB . Per the test report, the measured max. conducted output power is 27.52 dBm and max. measured ERP is 21.3 dBm. Please inform the max. designed output power is ERP or conducted . If the rated 0.25W is specified in term of conducted output power, please explain the different between rated output power with measured output power. Answer: : In the theory of operation, this phone is designed to have max. .25 W ( 24 dBm ) +2/-4 dB . The value is the conducted output power(Rated output power) by average. The conducted output power and the ERP in the Part 22 and SAR Test Reports are measured in Peak. The different between rated output power with measured output power is generated by the different of the measured detector. The AVERAGE Transmitter Power is added to Part22 Test Report(Pages 22 and 28). Please replace the theory of operation(Theoryof Operation_R.pdf) and Part22 Test Report(TestReport_Part22H_NRNA5514A_R.pdf). Question #2: During Body Worn SAR test, the phone was tested in the belt clip by positioning the camera away from the phantom. Can this phone be positioned in a reverse position with camera faced to the phantom? If yes, please provide SAR test data or justification for the positioned selected which representing the worse case. Answer: Transmitting antenna is located in the back panel of bottom of the device (see photograph). In our view, the hot spot is around this antenna position. Therefore we tested body-worn SAR that the back panel faced to the flat phantom. Question #3: Page 57 of user manual suggests this phone can be used with holster designed for this phone or any other accessories which can provide 1.5 cm separation distance. Please provide a phone to show the separation distance that the specific holster used during the tests and provide additional body worn SAR tests with air gap of 1.5 cm to demonstrate compliance with 1.5 cm separation distance. Answer: The data of the additional body worn SAR tests with air gap of 1.5 cm to demonstrate compliance with 1.5 cm separation distance described in the SAR Test Report(80050231R). Please replace the SAR Test Report(80050231_R.pdf) and the SAR Test Plot(2_SAR Test Plots_R.pdf). Question #4: Is there any metal part in the specific holster used during SAR tests ? What the part number of holster ? Answer: There are some metal parts specific holster using SAR tests. The part number of the holster is Part Number “0103FCA2”. Refer to the attached file” Optional Holster of A5514SA.pdf”. Antenna location

Output power listed is ERP. SAR compliance for body-worn operating configurations is limited to the specific configurations tested for this filing. Body-worn operations are restricted the specific belt-clips/holsters/accessories tested for this filing and to belt-clips, holsters or similar accessories that have no metallic component in the assembly and must provide at least 1.5 cm separation between the device, including its antenna whether extended or retracted, and the body of user. End-users must be informed of the body-worn operating requirements for satisfying RF exposure compliance. Highest reported SAR are : Head 0.359 W/kg; Body 0.528 W/kg.
