Safety and Regulatory The LT-LFS01-LM equipment has been tested and found to comply with the limits for a Class A digital device, pursuant to part 15 of the FCC Rules (its FCC ID is designated as PTD-EA-1121660). These limits are designed to provide reasonable protection against harmful interference in a residential installation. This equipment generates uses and can radiate radio frequency energy and, if not installed and used in accordance with the instructions, may cause harmful interference to radio communications. However, there is no guarantee that interference will not occur in a particular installation. If this equipment does cause harmful interference to radio or television reception, which can be determined by turning the equipment off and on, the user is encouraged to try to correct the interference by one or more of the following measures:  Reorient or relocate the receiving antenna.  Increase the separation between the equipment and receiver.  Connect the equipment into an outlet on a circuit different from that to which the receiver is connected.  Consult the dealer or an experienced radio/TV technician for help. Lumenis is not responsible for any radio or communication interference caused by using other than specified or recommended cables and or by unauthorized changes or modifications to this equipment. Unauthorized changes or modification could void the user’s authority to operate the equipment. This device complies with part 15 of the FCC Rules. Operation is subject to the following two conditions: 1. This device may not cause harmful interference, and 2. This device must accept any interference received, including interference that may cause undesired operation. This module also complies with the regulatory standard of ETSI EN 300 330-1 and ETSI EN 300 330-2, subjected to the SII test report No. 9212324456 regarding the LM- LFS01-LM electromagnetic compatibility and radio spectrum matters (ERM) for short LT-LFS01-LM Operator’s Manual UM-1146630, rev. A Page 7 range devices (SRD). Specifically, the operating frequencies, transmitter carrier output levels, radiated spurious emissions, etc. were found compatible to the standards are the module is permitted for use for within Lumenis systems and their intended use.

October 31, 2012 Statement The LogiRead LF one channel reader LT-LFS01-LM (FCC ID: PTD-EA-1121660) is an RFID integrator modified for Lumenis applications. The purpose of this module is mainly, but not limited to, the management of consumables attached to Lumenis systems. It has no use for the general public for any other application, and cannot be used as such, as it contains authentication keys known only to Lumenis. The RFID FCC ID: PTD-EA-1121660 module installation must be carried out by trained and authorized personnel. The module is intended to be employed within Lumenis' machines. Hence such an installation requires opening the laser console, which may be done by licensed professionals to begin with. Furthermore, the matching between the module and the system may vary from one machine to another and may require dedicated tools. The RFID module shall not be sold to the general public whatsoever. It must be sold to authorized distributers or service personnel as a spare part. Each sale and each installation is controlled and documented. After installation, the installer must verify the validity of installation, which may require specific knowledge, such as a service password to the machine or some service accessory, which is not a saleable item, but distributed to licensed professionals only. Sincerely, Eyal Benisty R&D, Electronics Manager Lumenis LTD.
