Reason why the Block Diagram, External Photos, Schematics, Test Data, Test Setup Photos, and Users Manual were upload twice: The reason why the above exhibits were uploaded twice is because one the first attempt to upload these exhibits, we did NOT receive a confirmation screen showing the exhibits had been uploaded successfully. Instead the website reset back to the OET main page without ever showing a confirmation. We were not sure whether these exhibits had uploaded, so we uploaded the above exhibits again just to make sure they were in the server. (This time we did get a confirmation screen showing they were uploaded). Any duplicate exhibit is the EXACT same file – just uploaded twice. Description of Use for REDEEM™ Transmitter Model 6000-4 The REDEEM™ transmitter is a professional incontinence management system designed to sense a fluid release from patients in nursing home or hospital. The transmitter (model 6000-4) is attached to a Sense’R Strip® that recognizes a wet condition in the patient’s bed. When a wet condition is detected, the transmitter is activated and sends a coded transmission to the companion REDEEM™ receiver (model 6000-3) located at the nurse’s station. The transmitter receives power from a 9-volt battery.

The test report is based on testing performed by Compatible Electronics personnel according to the measurement procedures described in the test specifications given below and in the "Test Procedures" section of this report. The measurement data and conclusions appearing herein relate only to the sample tested and this report may not be reproduced in any form unless done so in full with the written permission of Compatible Electronics. This report must not be used to claim product endorsement by NVLAP or any other agency of the U.S. Government. Device Tested: Transmitter Model: 6000-4 P/N: 30700 Product Description: See Expository Statement. Modifications: The EUT was not modified during the testing. Manufacturer: Health Sense International, Inc. 30211 Avenida de las Banderas, Suite 120 Rancho Santa Margarita, CA 92688 Test Date: November 9, 2000 File # For Canada: IC2154-D Test Specifications : EMI requirements CFR Title 47, Part 15 Subpart C, Sections 15.205, 15.209, and 15.249 Test Procedure: ANSI C63.4: 1992 Test Deviations: The test procedure was not deviated from during the testing. SUMMARY OF TEST RESULTS TEST DESCRIPTION RESULTS 1 Conducted RF Emissions, 450 kHz - 30 MHz This test was not performed because the EUT is powered by a three 3 volt lithium batteries. 2 Radiated RF Emissions, 10 kHz - 9300 MHz Complies with the limits of CFR Title 47, Part 15, Subpart C, sections 15.205, 15.209, and 15.249 FCC ID: OJC6000-W916RT02 Report No.: B01109D1 114 OLINDA DRIVE, BREA, CALIFORNIA 92823 PHONE: (714) 579-0500 FAX: (714) 579-1850 Page 5 of 16 1. PURPOSE This document is a qualification test report based on the Electromagnetic Interference (EMI) tests performed on the Transmitter Model: 6000-4. The EMI measurements were performed according to the measurement procedure described in ANSI C63.4: 1992. The tests were performed in order to determine whether the electromagnetic emissions from the equipment under test, referred to as EUT hereafter, are within the specification limits defined by CFR Title 47, Part 15, Subpart C, sections 15.205, 15.209, and 15.249.
