Important Information It is important for your facility to implement and enforce the following guidelines in order to keep all equipment functioning properly. User Training It is the responsibility of the facility to implement structured training procedures for all employees using the system. Only users who have received adequate training on the use of the system should use the system. Failure to adequately train employees may cause equipment failure due to user error. In addition, incorrect use of the equipment may also result in equipment failure. System Maintenance It is the responsibility of the facility to establish and facilitate a regular maintenance schedule of your (Safe Place ® ) system. This includes regular inspection, testing, and cleaning. Failure to provide regular maintenance to these products may result in equipment failure. Installation and Configuration It is the responsibility of the facility to use the supplies specified by RF Technologies for all (Safe Place ® ) installations. Failure to use the supplies specified by RF Technologies may result in equipment failure. RF Technologies, Inc. (herein referred to as “Seller”), warrants to the Buyer that the RF Technologies, Inc. (Code Alert/Safe Place/Sensatec) System (herein referred to as “Product”) will be free from defects in material and workmanship for a period of one (1) year from date of sale and title, and will conform to the Seller’s quotation. Transmitter battery life is one (1) year or as quoted for sale. Battery is covered by a limited warranty prorated over the life of the battery. The Battery warranty only applies to battery life and does not include casings or clamps, which is one (1) year as stated elsewhere in this warranty. This warranty is a limited warranty and it is the only warranty made by Seller. Seller’s sole obligation under this warranty shall be limited to repairing or replacing any part, which, within one(1) year from the date of sale, is returned to the factory, shipment prepaid, and upon examination by Seller, shall disclose to have been defective. Alternatively, Seller may, at its sole option, instead elect to refund the purchase price paid for the defective product. The criteria for all testing shall be Seller’s applicable product specifications utilizing factory specified calibration and test procedures and instruments. No allowance shall be made for local repair bills or expenses without the prior written approval of Seller. If Seller elects to replace any part, such replacement will not extend the life of this warranty. Seller reserves the right to use refurbished parts for warranty repairs or replacements. Buyer’s failure to pay the purchase price in full by the due date will void all warranties for the product. Warranty coverage does not include, and Seller disclaims any liability for, any defect or performance deficiency (including failure to conform to product descriptions or specifications) which results, in whole or in part, from (1) improper storage, handling, maintenance or installation of the Product by Buyer, its employees, agents, or contractors, (2) absence of any product, component, or accessory recommended by Seller, but omitted at Buyer’s direction, including but not limited to transmitters and banding materials not tested and approved, (3) any design, specification, or instruction changed by Buyer, its employees, agents, or contractors, (4) failure to comply with any applicable instructions or recommendations of Seller, including installation procedures, (5) physical damage occurring to transmitters or other components after receipt and acceptance by buyer, or (6) any components, systems, process, or equipment not sold or provided by Seller, or (7) acts of God, acts of civil or military authority, fires, floods, strikes, or other labor disturbances, war, riot, or other causes beyond the reasonable control of the Seller. Response to TCB Findings 1. Please supply a more detailed technical description of the transmitter circuitry. More detailed description supplied. 2. The label FCC ID is different to the Form 731. Correct label sample supplied. 3. The manual contains a RF exposure statement that is not necessary. Manual corrected. Please see revised manual. 4. Please describe the procedure you used to obtain the worst case 100ms transmission duration. Please see the document named “Response to TCB Finding - Item 4.pdf”.

Mother Baby Transmitter 317.8 - 318.2 MHz Transmitter Response to Item 4 In regards to the questions on the relaxation factor calculations in report #303443-Tx, the 2.16 ms should have been updated to 5.42ms and was not, due to an oversight on our part. The 2.16ms calculation for worst case on time was based on information provided to us, initially, on the 'Legacy format' protocol which I will describe later. After actually measuring the EUT, and discovering that we exceed the characteristics of the legacy format, the protocol document (see attached) was provided by RF Tech, and the worst case was re-examined and determined as a different format: the 'Long Format'. The re-examination never made it into the report. Please refer to the attached document from RF Technologies for the definitions of the bit symbols. These tests were performed using the test procedure outlined in ANSI C63.4, 2001 for intentional radiators, and in accordance with the limits set forth in FCC Part 15.231, for a periodic operation of a low power transmitter. 8. Purpose The tests were performed in order to determine the compliance of the EUT, with limits contained in various provisions of Title 47CFR, FCC Part 15, including: 15.205, 15.209 and 15.231(e).
