NOTICE TO OEM INTEGRATORS: *When this unit is placed inside of your host device, you must attach a label stating the following: "This device contains FCC ID: R5Y-EZWFM06ER" to the outside of the final product. FCC RF EXPOSURE INFORMATION WARNING! Read this information before using the device In August 1996 the Federal Communications Commission (FCC) of the United States with its action in Report and Order FCC 96-326 adopted an updated safety standard for human exposure to radio frequency electromagnetic energy emitted by FCC regulated transmitters. Those guidelines are consistent with the safety standard previously set by both U.S. and international standards bodies. The design of this device complies with the FCC guidelines and these international standards. Operating Requirements The antennas used for this transmitter must be installed to provide a separation distance of at least 20 cm from all persons and must not be co-located or operating in conjunction with any other antennas or transmitters. Please maintain 20 cm separation distance from the antenna to meet FCC RF ex posure compliance requirements. Specifically: In order to comply with FCC RF exposure requirements, OEM integrators must install this mini PCI card in notebook PCs with the antenna mounted inside or on the LCD display 20 cm from the end user. In addition, OEM integrators must inform end users of the following: “In order too comply with FCC RF exposure requirements, this device must be operated in such a way that a minimum 20cm separation distance is maintained between the antenna, and all persons, during normal operation.” FCC Radio Frequency Interference Requirements This product is restricted to indoor use due to its operation in the 5.15 to 5.25 GHz frequency range. FCC requires this product to be used indoors for the frequency range 5.15 to 5.25 GHz to reduce the potential for harmful interference to co-channel Mobile Satellite systems. High power radars are allocated as primary users of the 5.25 to 5.35 GHz and 5.65 to 5.85 GHz bands. These radar stations can cause interference with and/ or damage this product. EAZIX, Inc. Eazix, Inc. Unit 301 Plaz@ B, 6530 Northgate Avenue, Filinvest Corporate City, Alabang, Muntinlupa City, Philippines 1700 March 22, 2006 Federal Communications Commission Equipment Authorization Branch 7435 Oakland Mills Road Columbia, MD 21046 In re: EAZIX, Inc. FCC ID: R5Y-EZWFM06ER Modular Transmitter Approval Gentlemen: With regards to our application for equipment authorization of the Mini PCI module, listed below are the modular approval requirements with which the EUT complies: a. The modular transmitter has its own RF shielding. The shield is made of steel, tin coated. This is intended to ensure that the modules do not have to rely upon the shielding provided by the device into which it is installed in order for all modular coupling between the RF circuitry of the module and any wires or circuits in the device into which the module is installed. Such coupling may result in non-compliant operation. b. The modular transmitter has buffered modulation/data inputs to ensure that the module will comply with Part 15 requirements under conditions of excessive data rates or over-modulation. This is taken care of the Baseband Processor (BBP) chipset used. c. The modular transmitter has its own power supply regulation provided by voltage regulator ISL6414IR. This is intended to ensure that the module will comply with Part 15 requirements regardless of the power supplying circuitry in the device into which the module is installed. d. The modular transmitter complies with the antenna requirements or Section 15.203 and 15.204(c). The antenna employs a “unique” antenna coupler Mini Eazix, Inc. Unit 301 Plaz@ B, 6530 Northgate Avenue, Filinvest Corporate City, Alabang, Muntinlupa City, Philippines 1700 PCB between the cable and the antenna, and a proprietary connector between the cable and the EUT. A single antenna, as described in the filing, is the only one currently approved for use with the EUT. e. The modular transmitter has been tested in a stand-alone configuration, i.e. the module was completely uncovered inside the host pc during testing. This demonstrates that the module is capable of complying with Part 15 emission limits regardless of the device into which it is eventually installed. The transmitter module complies with the AC line conducted requirements found in Section 15.207. The antenna cable connected to the module does not contain ferrites. f. The modular transmitter is labeled with its own FCC ID number, and the OEM is instructed to provide an exterior label to be placed on the device into which the EUT is installed. g. There are no specific rules or operating requirements applicable to the transmitter with respect to user operation. h. The modular transmitter complies with the applicable RF exposure requirements, as specified in Section 15.247. The EUT is a mobile device. The installation instructions caution the user to install and operate the EUT in such a way that a minimum 20 cm separation distance is maintained between the antenna and all persons during normal operation. Sincerely, Arnel G. Lampa Senior Division Manager EAZIX, Inc.

Introduction This document attempts to prove the safety of radiation generated by RF devices to the human body. The limit for Maximum Permissible Exposure (MPE), specified in FCC 1.1210, is listed below. The power generated by this product is measured by a power meter. Through use of the Friis transmission formula and the maximum gain of the antenna, the distance from the product at which compliance with the MPE limit is achieved may be calculated. Alternatively, near field measurements may be performed to demonstrate compliance at a specific measurement distance. Near field probe: Wandel & Goltermann EMR-20. 2. RF Exposure Limit According to FCC 1.1310: the criteria listed in the following table shall be used to evaluate the environmental impact of human exposure to radio-frequency (RF) radiation as specified in 1.1307(b).
