Working Safely It is WaveMark’s highest priority that all field installations comply with the company’s installation and safety procedures and any safety procedures in place at the location of installation. In order to assure that WaveMark equipment is installed in a safe manner and in compliance with WaveMark’s installation guidelines only factory trained personnel are authorized to install or service WaveMark products. This WaveMark product contains no user-serviceable components. All personnel involved with the transportation, installation, commissioning or repair of WaveMark equipment are required to follow WaveMark’s Infection Control Practice policy (effective 12/2006). WaveMark, Inc. - Proprietary and Confidential 99-0002 Version 1.00 Page 5 of 11 2. Unpacking the Cabinet Step 1 When the cabinet arrives, it will need to be unpacked. WaveMark ships its cabinets on pallets with built-in ramps. Once the packing material has been removed from the outside of the cabinet, unlock the front wheels of the Cabinet, lower the ramp, and carefully guide the Cabinet down the ramp. Step 2 With the packing material removed inspect the cabinet for damage. If the Cabinet is obviously damaged notify the shipping company immediately. Don’t proceed with the installation of any damaged Cabinet. Step 3 Set the pallet aside and discard the rest of the shipping material as directed by the facility’s shipping personnel. When all Cabinets have been unpacked, stack the WaveMark pallets and coordinate with shipping personnel to return then to WaveMark’s designated storage facility; this is NOT WaveMark’s corporate address. Placing the Cabinet Step 1 Before moving a Cabinet up to its location in the facility, walk the route and note any obstructions such as signs on the walls or ceilings, or any flooring issues such as elevator thresholds that might be a problem for the Cabinet’s wheels. Step 2 Identify where the Cabinet is to be placed and locate an easily accessible socket outlet and the nearest usable Ethernet jack. The Cabinet MUST be connected to a power outlet that is properly grounded and has a three wire connection. Step 3 The Cabinet is designed to be rolled over smooth flooring; the cabinet should never be tilted, placed on its side, or rolled across parking lots. If necessary relocate the Cabinet on its shipping pallet to avoid these situations. When crossing elevator entrances use the portable threshold provided to prevent the Cabinet’s wheels from becoming lodged in the elevator entrance track. Note that the Cabinet has been designed to be very stable, but moving too quickly and pushing against the top of the Cabinet may cause it to tip. When placing the cabinet move slowly and push or pull only the lower half of the Cabinet. Step 4 Be sure the Cabinet is installed in a suitable environment. The environment should be free from airborne corrosive agents and excessive dust. The floor should be dry and the cabinet should not be placed in an area where it would be exposed to dripping water. Step 5 Inspect the exterior of the Cabinet and if it’s become marred during shipping clean the exterior plastics with a mild household cleaner or isopropyl alcohol. Other solvents that are more harsh may damage or discolor the Cabinet’s exterior and should NOT be used. WaveMark, Inc. - Proprietary and Confidential 99-0002 Version 1.00 Page 7 of 11 4. Cabinet Installation Step 1 Carefully wheel the cabinet to its location. Move the Cabinet slowly and carefully to avoid objects and people who may be traveling in the same corridors. Use the moveable threshold provided when traveling over rough spots, such as elevator entrances. Step 2 When the cabinet is in place, remove the top and bottom Cabinet Back pieces. Each antenna is held in place with a protective foam insert. Remove and discard the inserts for all the Antennas. Step 3 Remove the power cable from the cabinet bottom and set it aside; DO NOT connect power to the Cabinet at this time! Step 4 Check the location of the Mux board and the DCP PC, and all of the interconnect wiring. Be sure there are no loose, unconnected wires in the base of the Cabinet. Memo To: TCB From: Rick Desmarais Subj: WaveMark HF1000 Cabinet Date: February 9, 2007 All WaveMark products will only be installed by WaveMark trained personnel. Currently WaveMark has two field engineers on staff that perform field installations, upgrades and repairs. When shipped to an installation location the products may be removed from their packaging by hospital shipping/receiving personnel. However, are still site inspected, tested and brought online only by WaveMark trained personnel. WaveMark has created a simple release process in order to assure consistency in production and to manage product changes. The Engineering Change Board meets weekly to review, discuss and approve/deny product changes. Currently the Engineering Change Board members consist of the VP Engineering and Hardware Engineering Manager. It will be expanded to add other WaveMark personnel as the company grows. Hardware Engineering is currently responsible for manufacturing and inventory. Bills of materials as well as component specification sheets for all critical components are available at the receiving area. Incoming inspection is performed on all critical components. Purchasing is managed from an approved vendor list maintained by Hardware Engineering. All cabinet product builds have a documented assembly procedure that illustrates how each of the cabinet sub-assemblies is built and how an entire cabinet is completed using those sub-assemblies. The changes implemented at onsite testing have been incorporated into the build procedure as follows. 1. Each antenna board BOM has been updated to include the Steward part number LFB159079-000 (WaveMark P/N 41-0003) onto the circuit board, attached with tie wraps. WaveMark’s contract manufacturer has been given instructions on implementing this change. 2. Two Steward ferrites with part number 28A3851-0A2 (WaveMark P/N 41-0005) have been added to the BOM for the cabinet and the assembly manual includes instructions for attaching these to the two serial cables attached to the PC. 3. One Steward ferrite with part number 28A2029-0A2 (WaveMark P/N 41-0004) has been added to the BOM for the cabinet and the assembly manual includes instructions for attaching this to the the Mux to Motor board connection cable. 4. One Steward ferrite with part number 28A0593-0A2 (WaveMark P/N 41-0006) has been added to the BOM for the cabinet and the assembly manual includes instructions for attaching this to the floppy disk drive power cable extender that comes from the back of the PC. 5. A part number has been created and vendor specified for adhesive backed copper tape (WaveMark P/N 86-0014) and it has been added to the BOM. Instructions for attaching it to the cabinet just under the BNC connector supports for the antenna coax cable have been added to the assembly manual. 6. A part number has been created and vendor specified for conductive acrylic backed aluminum foil (WaveMark P/N 86-0015) with a width of two inches. Instructions for attaching it to the cabinet at the back of the PC have been added to the assembly manual.

We found that the product met the above requirements with modification (see Modifications Required for Compliance section on page 3). This report is an application in pursuit of a FCC grant for the HF1000 with FCC ID: UQY- HF1000 operating under 47CFR 15.225. The product was tested using the methods outlined in ANSI C63.4 (2003). The radio module used in the HF1000 was previously tested under FCC ID: PJMMR101-PR101.The manufacturer intends to use the following new antenna with the product. RFID Antenna “I” Detailed schematic of the antenna board is attached as an exhibit with this report. Fundamental was measured with a loop antenna at a distance of 30m from the product. Harmonics and spurious emissions were measured at 3m distance from the product. The EUT antennas could not be maximized separately. The EUT was only tested in floor standing orientation on a rotating turn table. All readings are peak unless otherwise specified on the respective table. Temperature and voltage variation tests were not performed on the radio sample because Wavemark has not made any changes to the original radio. Data presented in this report is from the radio’s original testing. Richard Desmarais from Wavemark was present during the testing. The test sample was received in good condition.
