La Habra, CA. 90631, United States E-MAIL : [email protected] Subject: Attestation for OPISYS Signal Booster FCC ID: Q4EAVHR-5000N Each Signal Booster certificated under FCC ID Q4EAVHR-5000N is manufactured and individually tested to ensure FCC compliance: (1) Licensee consent is not required for FCC certification equipment authorization purposes, but licensee consent does remain applicable per 20.21 provisions for both provider specific and wideband boosters. (2) NPS and other compliance/safeguard features have been implemented. (3) NPS and other compliance/safeguard features are defaulted to be “On” (in operation). (4) NPS and other compliance/safeguard features cannot be field reconfigured, disabled, or removed. (5) This consumer booster is not user programmable, does not need fine tuning or adjustment, and does not require professional installation. However, in case booster indicator lights show the need, the gain may be adjusted as described in the User Manual. Such adjustments are “fail safe”, i.e., they will not enable the booster to be non‐compliant to NPS or other compliance/safeguard requirements. (6) Future software upgrade will not cause non‐compliance. Sincerely Yours,

Power listed is conducted at antenna terminals. The antenna(s) used with this signal booster must be installed to provide a separation distance of at least 20 cm and must not be co-located or operating in conjunction with any other antenna or transmitter except in accordance with FCC multi-transmitter guidance. Antenna gain, less cable loss, must not exceed 5.0dBi and 2.0dBi for uplink and downlink antennas respectively; use of higher gain antennas will require submission of a C2PC to demonstrate compliance with radiated power limits, radiated spurious emissions, RF exposure requirements and the categorical exclusion requirements of §2.1091. Users and installers must be provided with antenna installation instructions and operating conditions for satisfying RF exposure compliance. This Consumer Signal Booster is authorized only for operation by and marketing to members of the general public for their personal use in accordance with the requirements of 47 CFR 20.21(a) and 20.21(g).
